what's new? Cosmetic Dermatology is evolving at a tremendously rapid rate. New procedures, new lasers and new injectibles have been introduced in the past year. Dysport, a Botox alternative was introduced in July; its effect and duration is similar to Botox with a more rapid onset of action. Evolence, a new injectible filler introduced in 2009, is known as 'the event filler' as side effects and bruising are minimal. Most patients can attend an event the next day. Some of the newest treatments to emerge include the 'liquid facelift,' hand rejuvenation, and pierced earlobe repair, which are outlined below. liquid facelift With aging, we experience a loss of volume in the face, which is especially pronounced in the mid-face. As midfacial fat descends, the nasolabial folds (smile lines around the nose and mouth) deepen, the cheeks hollow and we lose fullness over the cheekbones. In the past, the only way to address this problem was with plastic surgery, which is invasive and costly. However, the newest approach to remedy this problem is often referred to as a 'liquid facelift,' which involves dermal filler therapy, resulting in a replacement of volume on the apples of the cheeks and a natural cheek enhancement, which naturally elevates and smoothes the skin below. Compared to surgical procedures, dermal filler therapy at the midface is less invasive, with a lower risk of complication, and produces more natural results. Filler therapy has much less downtime and is more cost effective than surgery. A 'liquid facelift' involves the use of semi-permanent fillers, such as Sculptra (Poly-L-Lactic acid) and Radiesse (Calcium Hydroxylapatite). Semi-permanent fillers can actually stimulate collagen production; collagen is a naturally occurring protein with a supportive function that provides structure and support to various parts of the body including the skin. These fillers also provide volume, correct facial folds and wrinkles, and are long-lasting. They are recommended over permanent fillers, which do not adjust to changing facial shape and aging, and can become dated and obvious in the way of an aging plastic surgery face. More than any other cosmetic treatment, the success of a filler treatment is based on the artistic skill of the physician. To avoid having a young face, but old, 'experienced' hands, newest treatments focus on cosmetic rejuvenation of the hands. Treatments include laser resurfacing for superficial age and liver spots, and volume replacement with dermal fillers (either long-lasting hyaluronic fillers or semi-permanent fillers). ear lobe piercing repair With age and repeated wearing of heavier earrings, ear lobe piercings can become stretched and unsightly. This significantly contributes to aging. Physicians can now reduce the size of a stretched pierced hole of the earlobe with local filler injection. Best treated with semi-permanent, or long-lasting fillers. |
